Shared Care Pathway – £90 Registration Fee (includes VAT)

Our Shared Care Pathway is for UK clients aged 18+ who want their NHS GP to take on prescribing, with support and treatment oversight from our multidisciplinary team (MDT). This can make ongoing care more affordable while maintaining regular monitoring and professional oversight. 

 

Important:
GPs are increasingly requesting that clients are stable on private HRT for at least 6 months before they agree to Shared Care.
This ensures your care is safe, your GP has confidence in your treatment plan, and your monitoring is already well established.
 

You’ll begin by registering and following our prescribing support pathway for the first six months. After that, we’ll support you with the Shared Care request if it’s appropriate for your situation.


⚠ This pathway does require a GP who is willing to participate in shared care. If your GP is unsure, we can provide guidance on how it works.

Things to Know💡

The shared care pathway starts in the same way as our prescribing support pathway, with intake appointments, assessments, and blood tests. 

It's important to consider the ongoing costs of private blood tests and prescriptions, just in case your GP is not able or willing to support. 

Most GPs will only agree after seeing that:

  • You’ve had consistent prescribing
  • Regular blood tests and clinical reviews have taken place
  • You’re stable and well-monitored on your treatment

We strongly recommend speaking to your GP in advance to check if they would be open to shared care before starting this process. 

What Happens Next: The Prescribing Support Pathway

 Once your registration is complete,  Your journey starts with the Prescribing Pathway. Below is what that looks like, followed by the transition into Shared Care (if appropriate). 

Pathway Overview

Step 1: Initial Intake Appointment

Once registered, you'll be asked to complete a few forms and be provided with information around the use of off-label prescribing. It's important that you complete these as it provides the team with the information they need to support you.


You’ll meet with one of our experienced BPS registered psychologists to explore your goals, history, and support needs. This step helps ensure your care is safe, personalised, and in line with clinical and regulatory standards.


Two Appointment Options:


  • If you need a formal diagnosis:
    Book a Full Intake Appointment – £190.80 inc. VAT (60 minutes)
     
  • If you already have a formal diagnosis from a qualified psychologist:
    You may be eligible for a Transferral Intake – £91.20 inc. VAT (30 minutes)
     

To be eligible for the transferral intake, we’ll need you to upload a copy of your formal diagnosis so it can be reviewed by the clinical team. If it meets the required criteria for a diagnosis of gender incongruence, we’ll help you get booked in at a time that works for you.

Step 2: Blood Test Requests & Monitoring

Blood tests can now be arranged after your intake appointment, and should be completed before your clinical intake appointment is booked. 


They'll need to see up-to-date blood test results before your treatment plan can be discussed. These help keep your care safe and allow the team to make informed decisions.


You can arrange blood tests through:


  • Pride In Health (private referral – includes result analysis)
  • Your GP (if supportive – £33.60 referral letter + £13.20 result review)
  • A private clinic of your choice (£13.20 result review)
     

*Please note pricing is inclusive of VAT.

Step 3: Treatment Planning Appointment

Once your blood test results have been received and reviewed, you’ll book a Treatment Planning Appointment.


During this session, we’ll:

  • Review your goals and medical background
  • Provide information about the available options and monitoring requirements based on the best available evidence 
  •  Communicate your blood results and provide information about any further tests that may be advisable 
  • Support you in understanding informed consent and answer your questions
     

Please note: Prescribing decisions are not made during this appointment. All treatment plans and prescribing lines will be reviewed and authorised by a UK-registered clinician following test results and final prescribing review.

Step 4: Prescribing Review

This cost includes the cost of your appointment and the price of your initial prescription (normally £14.40 inc. VAT).


Once your test results are in and reviewed by the multidisciplinary team, you’ll have a short 10 minute appointment with a UK clinician.


This is your chance to:


  • Review your results with a UK clinician
  • Finalise your treatment plan
  • Ask any last-minute questions before your prescription is arranged


Ongoing treatment recommendations will be charged at £33.60 every 3 months inc. VAT.

Step 5: Ongoing Monitoring

You’ll continue on the prescribing pathway for at least 6 months, with:


  • Blood tests every 3 months
  • Monitoring appointments every 6 months
  • Prescriptions every 3 months, with regular review
     

This builds a clear, stable record of your care.

Step 6: Shared Care Agreement

After 6 months of safe, stable treatment, we will prepare a Shared Care Agreement (£33.60 inc. VAT) for your GP.

This will include:

  • Your treatment plan
  • A summary of your stability and monitoring
  • Contact details and clinical support framework

Step 7: Shared Care Requests & GP Follow-Up

 We’ll send the agreement directly to your GP practice and follow up where needed.
If accepted, your GP will begin prescribing, supported by our MDT.
If unsupported, you can continue receiving prescriptions privately with our ongoing support.

When you place a booking through our portal, it will be marked as provisional. 


An invoice will be sent to you within 24 working hours after you make your provisional booking (excluding weekends and bank holidays).  


Once you’ve received and paid your invoice, our team will review your payment and send you a confirmation email. At that point your booking is fully confirmed.  


After confirmation, you’ll be able to view, rearrange, or cancel your appointment directly through your Carebit portal.

 If your GP or private provider has offered blood tests, that’s no problem.
Just upload your results to your Carebit portal, and we’ll guide you on whether anything further is needed.
There’s a £14.40 inc. VAT results analysis fee if you’re not using our private partner labs.

Treatment Recommendation

As part of your Shared Care journey, the clinicians supporting you will provide treatment recommendations based on the latest clinical guidance, your personal goals, and your test results. These recommendations are shared with your GP as part of the Shared Care Agreement to help ensure your ongoing NHS prescribing is safe, consistent, and supported.


Recommendations may include details such as suggested medication and dosage, preferred route of administration, baseline and follow-up testing schedules, and any additional clinical considerations (such as the need for blood pressure monitoring or potassium levels depending on your chosen medication). These are designed to offer your GP a clear and structured plan to follow.

Your GP always remains in control of what they choose to prescribe, but our team will support them with clear, evidence-informed information and can be contacted by them if they have any questions.

Shared Care Pricing

Intake

These are the starting steps required to begin your journey with the prescribing team.

1. Registration

£75 + VAT (£90.00)

Covers admin, document review, and client setup. Gives you full access to the Carebit portal. 

2. Intake Appointment (Transferral)

£76 + VAT (£91.20)

For clients who already have a formal diagnosis from a qualified psychologist. A short 30 minute appointment to check your history and goals. 

3. Intake Appointment Including Formal Diagnosis

£159 +VAT (£190.80)

For those without a diagnosis. A 60 minute appointment with a psychologist exploring your gender journey, support needs, and eligibility for formal diagnosis if appropriate.

4. Full Health Check & Hormone Baseline

£173 + VAT (£207.60)

Required at the start of care and yearly. Includes full blood count, liver function, kidney function (U&Es), lipids, hormone levels (testosterone & oestradiol), and general health markers. If your GP is supportive at this early stage, a blood test referral can be sent to your GP to conduct the tests. Reducing the cost to £24 for the letter.

5. Treatment Planning Appointment

£157 + VAT (£188.40)

A 45 minute appointment to discuss your care goals, test results, and treatment options. Prescriptions are not issued at this stage, and no clinical decisions will be made at this stage.

6. Prescribing Review

£56 + VAT (£67.20)

A 10 minute appointment with a UK-registered prescriber. Includes your first treatment recommendation (£24) and final treatment check. Following the appointment, your prescription will be created (and a reference copy shared in your portal), please note the prescription won't be released until your file and prescription has had a final audit check by another of our UK registered prescribing clinicians.


Ongoing Monitoring Protocols

As part of the Prescribing Support Pathway, we use regular blood testing to help keep your care safe and responsive. The tests listed below are used at key points (typically every 3 months in the first year) depending on your medication and personal needs. After 12 months on a stable dose, your monitoring schedule may be reduced to 6 monthly or yearly. Please note that the blood test pricing in this section is for the tests to be completed privately, If your GP is supportive, your tests will be

Hormone Monitoring Panel

£88 + VAT (£105.60)

Used at Month 3, 6, 9, and 12
Who it's for: Anyone taking oestrogen, testosterone, or other gender-affirming hormones


What's tested: Oestradiol & testosterone levels (depending on pathway).


Why it's important: Helps check that your hormone levels are within a safe and effective range 

Hormone Monitoring Panel + Full Blood Count (inc. Hb & Hct)

£102 + VAT (£122.40)

Used at Month 3, 6, 9 for clients on testosterone

Who it's for: Those prescribed testosterone or on masculinising hormone therapy


What's tested: Hormone levels, haemoglobin, and haematocrit


Why it's important: Testosterone can increase red blood cell count, so this test checks for any risks like thickened blood 

Hormone Monitoring Panel + Liver Function Test + Blood Pressure

£102 + VAT (£122.40)

Used at Month 3, 6, 9 when taking Cyproterone
Who it's for: Clients on Cyproterone with oestrogen

What's tested: Hormone levels and liver function (ALT, AST, bilirubin), with a record of blood pressure


Why it's important: Cyproterone can affect the liver, so this combination checks for early signs of impact 

Hormone Monitoring Panel + U&Es (Potassium)

£102 + VAT (£122.40)

Used at Month 3, 6, 9 when taking Spironolactone
Who it's for: Clients on Spironolactone

What's tested: Hormone levels, urea, creatinine, electrolytes (especially potassium).

Why it's important: Spironolactone can raise potassium, which can be dangerous if not properly monitored. Potassium tests should be taken as a same day sample to ensure an accurate reading. For more information please reach out via services@prideinhealth.co.uk.

Full Health Check + Hormone Panel

£173 + VAT (£207.60)

Used at Baseline and again at Month 12
Who it's for: All clients starting or continuing gender-affirming hormone therapy

What's tested: Hormone levels, full blood count, liver and kidney function, cholesterol, and general health screening

Why it's important: Gives a complete picture of your health before starting treatment and after 12 months to assess long-term safety

6-Monthly Monitoring Appointment (with UK Clinician)

£92 + VAT (£110.40)

Includes: Appointment + prescription (usually £12)
When it's used: Every 6 months while on hormone therapy

Who it's for: Anyone receiving prescriptions through our Prescribing Support Pathway

What happens:

  • Review of your blood test results
  • Monitoring for side effects, wellbeing, and treatment response
  • Update to your treatment plan (if needed)
  • Renewal of your prescription for the next 3 months
     

Why it matters: This appointment is an essential check-in with a UK-registered clinician to make sure you

Blood Test Analysis (Results Only)

£11 + VAT (£13.20)

Used when submitting results from an external provider
Who it's for: Clients who choose to get bloods done via their GP or private lab

What's included: Review by the clinical team to ensure your results meet prescribing criteria

Why it's important: Ensures the team can assess whether treatment remains safe based on third-party results 


Counselling Support Appointments

Work with gender-affirming mental health professionals who understand the trans and non-binary experience. You can use appointments for general support, transition-related wellbeing, or anything else on your mind.

Counselling/Support 25min

£67 + VAT (£80.40)

A shorter consultation ideal for check-ins or ongoing support. Delivered by gender-affirming therapists from our community. 

Counselling/Support 60min

£92 + VAT (£110.40)

A full-length therapy session for more in-depth emotional support, transition guidance, or mental health needs. 


Clininal Appointments & Letters

Speak with clinicians who understand gender-affirming care. Appointments are available to support you with safe self-management, advice, and access to letters.

Gender Marker Change Letter

£69 + VAT (£82.80)

A short appointment and letter to support updating your gender marker with the passport office, or other services. Please note this  is not the same as a gender recognition certificate (GRC). If you're looking for a GRC, please do reach out and we'll do our best to support you with information and resources.

Surgery Referral

£156 + VAT (£187.20)

Includes a clinical consultation and preparation of a referral letter for private or NHS surgery (top, bottom, or facial surgery).

Blood Test Request Letter

£28 + VAT (£33.60)

A prepared letter for your GP asking them to carry out hormone-related blood tests. You’ll need to confirm with your GP before requesting this letter.

Injection Request Letter

£28 + VAT (£33.60)

Used to request that your GP take over administration of hormone injections. You’ll need to confirm with your GP before requesting this letter. 

Shared Care Request Letter

£28 + VAT (£33.60)

What it includes:

  • A formal request letter written by the multiciliary team (MDT) and signed by a UK registered clinician
  • Submission of the shared care request to your GP
  • Option for a guided editable letter for you to send to your GP yourself prior to paying the fee and sending the shared care request letter.

What it’s for:
This letter can be used if you wish to explore shared care with your NHS GP. It formally asks your GP consider prescribing and monitoring as part of a shared care arrangement. 

Important to know: Shared care request letters do not guarantee your GP will agree to shared care. The decision rests with your individual GP or practice, and many have their own internal policies. We strongly recommend speaking with your GP in advance to understand whether they’re open to shared care.

Frequently Asked Questions

Shared Care allows the independent clinician working through Pride In Health, to provide ongoing support, while your NHS GP prescribes your medication. This approach helps reduce costs while ensuring continuity of care.


You must already have an established treatment plan and stable prescribing history with our clinicians or another recognised provider. You’ll also need to provide your GP’s contact details for communication and consent purposes. 


Yes, you’ll need to complete our Prescribing Pathway first so we can establish safe, ongoing oversight before your GP is asked to prescribe. 


Shared Care registration is £90 (including VAT). This covers account setup, document verification, and coordination with your GP.

Once you’ve had stable HRT prescribing and regular monitoring for 6 months, we can send your GP a shared care request letter. 


You will, your GP needs to be willing to participate in shared care. If they’re unsure, we can help by providing a shared care agreement (£33.60 inc. VAT) with information on how it works and answering any questions they may have. 


Yes, you can continue to receive private prescriptions through our team, and we’ll keep supporting you however you need. 


Unfortunately, some GPs aren’t comfortable with shared care, even when guidance supports it. If your GP refuses, you might want to look into the Comprehensive Care Pathway, which offers private prescribing instead.


We know this can be frustrating, but you’re not alone, many of us have been there too. If you need help figuring out what to do next, we’re here to help talk through your options.


Once you register and submit your documents, we aim to review everything within 2 working days and confirm your registration by email along with an invoice for your registration. Once paid, we will get you set up within the system so you can then start booking your appointments. 


A prescription is a legal document that allows a pharmacy to dispense medication.
A treatment recommendation is a formal clinical document sent to your GP under Shared Care, outlining the medication, dosage, and monitoring schedule. Your GP uses this to support their own prescribing decision. 


No, we do not dispense medication. Under Shared Care, your GP issues the prescription, and you’ll collect your medication from your local pharmacy. 


You can remain under Prescribing Support until shared care can be safely arranged. The clinical team will continue monitoring and issue prescriptions directly through SignatureRX during this time.It's important to be prepared to take on the financial responsibility for private support. You can find our pricing throughout our website, and speak to your local pharmacy for details of the medication costs.


These details help your clinician make sure that any treatment or medication prescribed is safe and suitable for you. Hormones and other medicines can affect things like blood pressure, weight, and metabolism, so having this information means your care can be monitored safely over time. It also helps identify if any adjustments might be needed to keep you feeling well throughout your journey. 


You can measure your height and weight at home using a tape measure and bathroom scales, or ask your GP surgery or local pharmacy if you don’t have access to these.

Blood pressure can be checked at most pharmacies, GP surgeries, or using a home monitor. If you’re using your own monitor, please make sure you’re seated and relaxed before taking your reading.
