This pathway is for our trans, non-binary, and intersex community in the UK (aged 18+) who have a supportive GP and want a collaborative approach to their gender-affirming care.
With shared care, you’ll have access to our multidisciplinary team (MDT) of gender-affirming clinicians while your GP manages your prescriptions. This means you get ongoing medical oversight, individualised treatment plans, and expert support, all while keeping your care connected to your local healthcare provider.

Once registered, you'll be asked to complete an intake form, clinical intake form and consent to sharing information form. It's important that you complete these as it provides the team with the information they need to support you.
If you need a formal diagnosis, you’ll book an intake appointment with a psychologist:
This appointment will provide information around gender incongruence, discuss your transition goals, and assess your capacity to consent.
After your initial intake appointment, you’ll book a 45-minute consultation (£136) with a doctor to discuss your transition goals.

Shared care means that your GP prescribes your gender-affirming medication, while the independent team provide ongoing medical oversight and recommendations. This allows you to keep your care connected to NHS services while still accessing expert support.
You will, your GP needs to be willing to participate in shared care. If they’re unsure, we can help by providing a shared care agreement (£24) with information on how it works and answering any questions they may have.
Unfortunately, some GPs aren’t comfortable with shared care, even when guidance supports it. If your GP refuses, you might want to look into the Comprehensive Care Pathway, which offers private prescribing instead.
We know this can be frustrating, but you’re not alone, many of us have been there too. If you need help figuring out what to do next, we’re here to help talk through your options.
Once you register and submit your documents, we aim to review everything within 2 working days and confirm your registration by email along with an invoice for your registration. Once paid, we will get you set up within the system so you can then start booking your appointments.
This depends on your GP’s approval process. Once your Shared Care Agreement is accepted, the next step is to arrange your blood tests, have them reviewed by the multidisciplinary team and creating your treatment recommendation. From there, your GP can start prescribing. We’ll continue to support you with treatment recommendations (£12) every 3 months to keep things moving smoothly.
If at any point you decide to move to Comprehensive Care for private prescribing, you won’t have to re-register, you’ll just pay the difference in registration fees.
If your GP later refuses shared care, you can also switch to private prescribing without losing your progress.
