Shared Care

Shared Care Pathway – £65 Registration Fee

Our Shared Care Pathway is for UK clients aged 18+ who want their NHS GP to take on prescribing, with support and treatment oversight from our multidisciplinary team (MDT). This can make ongoing care more affordable while maintaining regular monitoring and professional oversight. 

 

Important:
GPs are increasingly requesting that clients are stable on private HRT for at least 6 months before they agree to Shared Care.
This ensures your care is safe, your GP has confidence in your treatment plan, and your monitoring is already well established.
 

You’ll begin by registering and following our prescribing support pathway for the first six months. After that, we’ll support you with the Shared Care request if it’s appropriate for your situation.


⚠ This pathway does require a GP who is willing to participate in shared care. If your GP is unsure, we can provide guidance on how it works.

Things to Know💡

The shared care pathway starts in the same way as our prescribing support pathway, with intake appointments, assessments, and blood tests. 

It's important to consider the ongoing costs of private blood tests and prescriptions, just in case your GP is not able or willing to support. 

Most GPs will only agree after seeing that:

  • You’ve had consistent prescribing
  • Regular blood tests and clinical reviews have taken place
  • You’re stable and well-monitored on your treatment

We strongly recommend speaking to your GP in advance to check if they would be open to shared care before starting this process. 

What Happens Next: The Prescribing Support Pathway

 Once your registration is complete,  Your journey starts with the Prescribing Pathway. Below is what that looks like, followed by the transition into Shared Care (if appropriate). 

Pathway Overview

Step 1: Initial Intake Appointment

Once registered, you'll be asked to complete a few forms and be provided with information around the use of off-label prescribing. It's important that you complete these as it provides the team with the information they need to support you.


You’ll meet with one of our experienced BPS registered psychologists to explore your goals, history, and support needs. This step helps ensure your care is safe, personalised, and in line with clinical and regulatory standards.


Two Appointment Options:


  • If you need a formal diagnosis:
    Book a Full Intake Appointment – £138 (60 minutes)
     
  • If you already have a formal diagnosis from a qualified psychologist:
    You may be eligible for a Transferral Intake – £66 (30 minutes)
     

To be eligible for the transferral intake, we’ll need you to upload a copy of your formal diagnosis so it can be reviewed by the clinical team. If it meets the required criteria for a diagnosis of gender incongruence, we’ll help you get booked in at a time that works for you.

Step 2: Blood Test Requests & Monitoring

Blood tests can now be arranged immediately after your intake appointment, and should be completed before your clinical intake appointment is booked. 


They'll need to see up-to-date blood test results before your treatment plan can be discussed. These help keep your care safe and allow the team to make informed decisions.


You can arrange blood tests through:


  • Pride In Health (private referral – includes result analysis)
  • Your GP (if supportive – £24 referral letter + £9 result review)
  • A private clinic of your choice (£9 result review)

Step 3: Treatment Planning Appointment

Once your blood test results have been received and reviewed, you’ll book a Treatment Planning Appointment.


During this session, we’ll:

  • Review your goals and medical background
  • Provide information about the available options and monitoring requirements based on the best available evidence 
  •  Communicate your blood results and provide information about any further tests that may be advisable 
  • Support you in understanding informed consent and answer your questions
     

Please note: Prescribing decisions are not made during this appointment. All treatment plans and prescribing lines will be reviewed and authorised by a UK-registered clinician following test results and final prescribing review.

Step 4: Prescribing Review

This cost includes the cost of your appointment and the price of your initial prescription (normally £12).


Once your test results are in and reviewed by the multidisciplinary team, you’ll have a short 10 minute appointment with a UK clinician.


This is your chance to:


  • Review your results with a UK clinician
  • Finalise your treatment plan
  • Ask any last-minute questions before your prescription is arranged


Ongoing prescriptions will be charged at £12 every 3 months.

Step 5: Ongoing Monitoring

You’ll continue on the prescribing pathway for at least 6 months, with:


  • Blood tests every 3 months
  • Monitoring appointments every 6 months
  • Prescriptions every 3 months, with regular review
     

This builds a clear, stable record of your care.

Step 6: Shared Care Agreement

After 6 months of safe, stable treatment, we will prepare a Shared Care Agreement (£24) for your GP.

This will include:

  • Your treatment plan
  • A summary of your stability and monitoring
  • Contact details and clinical support framework

Step 7: Shared Care Requests & GP Follow-Up

 We’ll send the agreement directly to your GP practice and follow up where needed.
If accepted, your GP will begin prescribing, supported by our MDT.
If unsupported, you can continue receiving prescriptions privately with our ongoing support.

Even if you plan to use shared care, it’s essential to be prepared for the possibility of your GP declining support.


We recommend having a backup plan and ensuring you can afford:


  • Private blood tests
  • Private prescriptions
  • Ongoing monitoring appointments

Frequently Asked Questions

Shared Care is when your NHS GP agrees to prescribe your hormone treatment, based on clinical oversight and recommendations from our team. 


This pathway is for trans, non-binary, and intersex adults who want long-term prescribing through their GP, with support from our MDT. 


Yes, you’ll need to complete our Prescribing Pathway first so we can establish safe, ongoing oversight before your GP is asked to prescribe. 


Once you’ve had stable HRT prescribing and regular monitoring for 6 months, we can send your GP a shared care request letter. 


You will,  your GP needs to be willing to participate in shared care. If they’re unsure, we can help by providing a shared care agreement (£24) with information on how it works and answering any questions they may have. 


Yes. You can continue to receive private prescriptions through our team, and we’ll keep supporting you however you need. 


Unfortunately, some GPs aren’t comfortable with shared care, even when guidance supports it. If your GP refuses, you might want to look into the Comprehensive Care Pathway, which offers private prescribing instead.

Once you register and submit your documents, we aim to review everything within 2 working days and confirm your registration by email along with an invoice for your registration. Once paid, we will get you set up within the system so you can then start booking your appointments.
