Before you see a prescribing clinician, you’ll now have a Care Navigation appointment. It’s non clinical, community informed, and it’s there to help you get everything in place without you having to chase things or second guess what’s next.

More clinician time where it matters
Because more of the prep happens earlier, the clinician appointment can focus on you, your health, your goals, your options, and making sure any treatment decisions are safe and properly informed. It should feel less rushed, and more like you’ve actually had the time and space to talk things through.
Fewer avoidable delays
Baseline blood tests are now arranged and completed before the first clinician appointment, so results can be timed correctly and properly reviewed in that appointment. That should mean fewer “we can’t move forward yet” moments, and less chance of you having to book extra appointments just because something was missing.
Shared care is now blended into Prescribing Support
Our aim is to support as many people as we can into shared care, to help reduce costs over time and improve GP support. In most cases, shared care can be requested after 6 months with us, this helps build GP trust and shows a clear, well monitored plan. You’ll also receive a Shared Care Pack to guide you through the process.

A structured, informed-consent-based pathway for trans, non-binary, and intersex adults in the UK seeking safe and personalised access to gender-affirming hormone treatment.
This pathway offers support through our multidisciplinary team (MDT) of Hormone Specialists, Nurses, Pharmacists, Supervising Clinical Psychologists, Psychologists, and and experienced transgender healthcare navigation support, providing you access to hormone support tailored to your needs.
This pathway requires clinical oversight, blood tests, and regular review to help UK-registered clinicians ensure the safety of any prescribing decisions.

This is the clinical appointment where your medical history is reviewed, your goals are explored properly, risks and benefits are discussed, and informed consent is checked. If baseline bloods are present and it is clinically appropriate, treatment may be initiated from this point.

Shared care at 6 months, if things are stable and well monitored
After 6 months of safe, stable treatment with consistent monitoring, you can request shared care. If your GP accepts, they may take on prescribing while you continue to access structured monitoring support as needed. If they do not accept, you can continue privately so you are not left stuck.

Prescriptions are issued by independent UK registered prescribers and sent to you via SignatureRx medication costs are not included and are paid directly to the pharmacy you choose.

After registration, everything is managed through your secure Carebit portal. You’ll be able to:

Just upload your results to your Carebit portal, and we’ll guide you on whether anything further is needed.
There’s a £13.20 results analysis fee if you’re not using our private partner labs.
Prescriptions are issued digitally using SignatureRx. You’ll receive a secure message or link you can take to a pharmacy of your choice.
We can also liaise directly with the pharmacy if any issues arise.
These are the starting steps required to begin your journey with the prescribing team.
Covers admin, document review, and client setup. Gives you full access to the Carebit portal.
For clients who already have a formal diagnosis from a qualified psychologist. A short 30 minute transferral appointment to check your history and goals.
For those without a diagnosis. A 60 minute appointment with a psychologist exploring your gender journey, support needs, and eligibility for formal diagnosis if appropriate.
A 30 minute appointment. This is here to help you feel supported and organised before the clinical steps. We’ll make sure key forms and consent bits are issued for completion, share pre clinician general resources, capture GP details where possible, and confirm readiness tasks. No clinical advice or interpretation is provided, anything clinical is escalated to the clinical team.
Full Health Check and Hormone Panel. These need to be arranged and completed before Step 4. This panel includes full blood count, kidney function, liver function, lipids, diabetes screening (HbA1c), prolactin, and hormones (oestradiol and testosterone).
This is your main clinical appointment. Your clinician will review your medical history, discuss fertility and signposting, talk through treatment options, risks and benefits, confirm your monitoring plan, and document the informed consent discussion. Your baseline blood results must be present and interpreted before progressing. Where clinically appropriate, treatment may be initiated at this step, with treatment specific resources shared after clinician sign off.
As part of the Prescribing Support Pathway, we use regular blood testing to help keep your care safe and responsive. The tests listed below are used at key points (typically every 3 months in the first year) depending on your medication and personal needs. After 12 months on a stable dose, your monitoring schedule may be reduced to 6 monthly or yearly.
Used at Month 3, 6, 9, and 12
Who it's for: Anyone taking oestrogen, testosterone, or other gender-affirming hormones
What's tested: Oestradiol or testosterone levels (depending on pathway), plus SHBG and LH/FSH when needed
Why it's important: Helps check that your hormone levels are within a safe and effective range
Used at Month 3, 6, 9 for clients on testosterone
Who it's for: Those prescribed testosterone or on masculinising hormone therapy
What's tested: Hormone levels, haemoglobin, and haematocrit
Why it's important: Testosterone can increase red blood cell count, so this test checks for any risks like thickened blood
Used at Month 3, 6, 9 when taking Cyproterone
Who it's for: Clients on Cyproterone with oestrogen
What's tested: Hormone levels and liver function (ALT, AST, bilirubin), with a record of blood pressure
Why it's important: Cyproterone can affect the liver, so this combination checks for early signs of impact
Used at Month 3, 6, 9 when taking Spironolactone
Who it's for: Clients on Spironolactone
What's tested: Hormone levels, urea, creatinine, electrolytes (especially potassium).
Why it's important: Spironolactone can raise potassium, which can be dangerous if not monitored.
Used at Baseline and again at Month 12
Who it's for: All clients starting or continuing gender-affirming hormone therapy
What's tested: Hormone levels, full blood count, liver and kidney function, cholesterol, and general health screening
Why it's important: Gives a complete picture of your health before starting treatment and after 12 months to assess long-term safety

A short appointment and letter to support updating your gender marker with the passport office, or other services. Please note this is not the same as a gender recognition certificate (GRC). This letter can be used if you wish to explore shared care with your NHS GP. It formally asks your GP consider prescribing and monitoring as part of a shared care arrangement.
Important to know: This letter does not guarantee your GP will agree to shared care. The decision rests with your individual GP or practice, and many have their own internal policies. We strongly recommend speaking with your GP in advance to understand whether they’re open to shared care.
The Prescribing Pathway is for those starting, continuing, or re-establishing gender-affirming hormone therapy with one of our independent clinicians. This pathway includes structured appointments, blood tests, and regular monitoring.
For trans, non-binary, and intersex adults (18+) who want access to hormone support with medical oversight, ongoing monitoring, and safe prescribing from UK-registered clinicians.

You can continue to receive prescribing support through our independent clinicians. The team will ensure your safety through structured monitoring and follow-up reviews. Please note it is important to ensure you can afford the onboarding and ongoing monitoring costs, and speak to your local pharmacy to understand the cost of medication.
No, the clinical team work under an informed consent model, so you don’t need a diagnosis to start your journey with us, but a formal diagnostic assessment is included with your intake appointment.
If you already have a diagnosis, we’ll just need to verify it with a short intake appointment (£91.20). If you don’t have one but need one, we offer access to a longer assessment (£190.80) to provide the right documentation.
Yes, you will, blood tests are essential for safe prescribing. These are usually done every three months initially, then every six or twelve months once stable. Prices vary from £92.40 to £207.60 depending on your medication and the related panels required.
No, we don’t hold or dispense any medication. Prescriptions are issued digitally and sent to you directly via SignatureRx. You can take it to your preferred pharmacy for sdispensingwho forward them to your chosen pharmacy for dispensing.
Once you’ve submitted your documents, we’ll generally review them within 2 working days and send your invoice for the registration fee (£90) and once paid, confirm approval with confirmation of your Carebit portal account. From there, you can start by booking your intake appointment and accessing the support you need. We’ll keep you updated along the way, so you’re not left wondering what’s next.
We aim to process registrations within 5 working days. If there are any delays, we’ll always keep you in the loop, we understand waiting is hard when you’re just trying to move forward with your care.
If at any point you decide you don’t need prescribing support anymore, you can switch to Flexible Support without having to re-register. Your access to things like blood test referrals, therapy, and letters will continue until you choose to close your account.
If you started with Flexible Support and have chosen to access support with prescribing, you’ll just pay the difference in registration fees and won’t have to go through the full process again. We want to make this as simple and stress-free as possible.
These details help your clinician make sure that any treatment or medication prescribed is safe and suitable for you. Hormones and other medicines can affect things like blood pressure, weight, and metabolism, so having this information means your care can be monitored safely over time.
It also helps identify if any adjustments might be needed to keep you feeling well throughout your journey.
You can measure your height and weight at home using a tape measure and bathroom scales, or ask your GP surgery or local pharmacy if you don’t have access to these.

Blood pressure can be checked at most pharmacies, GP surgeries, or using a home monitor. If you’re using your own monitor, please make sure you’re seated and relaxed before taking your reading.
