Ongoing care supports people after starting gender-affirming hormone treatment, with regular clinical review, blood test monitoring, prescribing, shared care guidance and support based on clinical need.
This helps make sure treatment remains safe, appropriate and responsive to your goals over time.
What ongoing care can include

With this pathway, blood tests are arranged through Pride In Health as part of your ongoing care.
This option suits people who want a more supported route for blood testing and review, with less admin to manage themselves.

With this pathway, you arrange blood tests separately and send the results to us as a PDF for review.
This option suits people who already have access to blood tests elsewhere and are comfortable arranging and sending results themselves.

With shared care, your GP may issue prescriptions and arrange blood tests, while we continue clinician-led review, treatment recommendations, documentation and communication where needed to support safe care.
Shared care is not part of intake. It is only available to request after at least 6 months of well-monitored care, and only where your GP agrees.
A monthly payment option is available for people who would prefer to spread the cost of ongoing care more predictably.
This is not a subscription. It does not require you to commit to paying for a set number of months or years, and it does not change what is included in your gender-affirming care.
If you need to pause or stop your care, monthly payments can be suspended until you are ready to access services and support again.

A GP may issue prescriptions and arrange blood tests, while we continue clinician-led review, treatment recommendations, and communication where needed.
Shared care is not a lower support pathway. Although your GP may issue prescriptions and arrange blood tests, this pathway still involves ongoing clinician-led oversight, treatment planning, documentation, and communication with yourself and your GP to support safe care.
Shared care is only available after at least 6 months of well monitored care and where your GP agrees.

Ongoing care is there to help treatment stay safe, practical, and well supported over time.
What this usually includes:

After at least 6 months of stable, well-monitored care, you can request shared care.
If your GP agrees, they may take on prescribing and blood tests, while you continue to receive clinician-led review, treatment recommendations, documentation and support from Pride In Health.
If your GP does not agree, you can continue privately so you are not left without care.

Shared care is always at the discretion of the GP practice, even when everything is stable. It is worth speaking to your GP early, so you know what may be realistic in your area.
If you are already registered with Pride In Health, you can manage your ongoing care through your Carebit portal. You can use your portal to book appointments, upload blood test results, view records and keep your information up to date.
Where prescriptions are issued through Pride In Health, they are sent digitally using SignatureRx. Medication and dispensing costs are charged separately by the pharmacy. If you are in shared care, your GP may issue prescriptions where they have agreed to do so.
If your GP or an external provider can arrange your blood tests, you can upload the results to your Carebit portal as a PDF. We will guide you on whether anything further is needed before clinical review.
Your ongoing care fees support clinician-led review, treatment planning, clinical documentation, blood test review where included, prescription review where clinically appropriate, shared care communication and the systems needed to keep care safe and well organised.
Pride In Health is a not-for-profit organisation.
This means any surplus is reinvested back into developing, improving and expanding the gender-affirming care and ongoing support we are able to offer.

These are ways of explaining how ongoing care often develops over time. In most cases, the main difference is the frequency of blood test review. Movement between them depends on clinical need, and not everyone will move at the same pace.
